Adult Program $299/year, age 14 and older

• 2 Adult periodic exams
• 2 Routine cleanings/prophylaxis
• 1 Set of digital bitewing x-rays
• 1 Limited exam
• 15% off routine restorative care

Children's Program $249/year, age 13 and younger

• 2 Child periodic exams
• 2 Routine cleanings/prophylaxis
• 1 Set of digital bitewing x-rays
• 1 Fluoride treatment
• 1 Limited exam
• 15% off routine restorative care

Adult Periodontal Program $475/year

• 2 Adult periodic exams
• 3 periodontal maintenance cleanings
• 1 set of digital bitewing x-rays
• 1 Limited exam
• 15% off routine restorative care
